Form 4506-A is an IRS form used to apply for an SSN. The form is used by the IRS to validate the number in order to prevent fraudulent SSNs from being issued. To fill out the form, one must fill out the blanks in the 'Information for Individual' section with their name, date of birth, and address. The form also asks for information about the individual's parents, such as the mother's and father's name and date of birth. The next section of the form is for the applicant's spouse and/or children. This section also includes the date of birth and the social security number. The next section is for identifying information, such as the applicant's race and ethnicity, and whether or not he or she has ever applied for an SSN. The next section is for the applicant's state of residence. The last section is for the applicant's signature. Once the form is filled out, it is mailed to the IRS.
