Alternative Form: 4506-EZ

Form 4506-EZ is used to request an extension of time to file a return or pay taxes. A taxpayer can use this form if they are unable to file their tax return or pay their taxes by the deadline due to a casualty, disaster, or other event.

The form includes documentation of the event that prevented the taxpayer from meeting the deadline. The taxpayer should attach documentation to the form, which should include a letter explaining the event, police reports, insurance reports, etc.

If the IRS approves the request, the taxpayer will have an additional six months to file their return or pay their taxes. During this time, they may be eligible to receive tax relief.
